  1. Download the New Patient packet, print it out, and bring the completed packet to your appointment

  2. Download the New Patient packet, fill out electronically, and email to cscheel@kwanyinhealingarts.com with your scheduled appointment date.

  3. Come 30 minutes early to your appointment to fill out paperwork before your visit

Third: Gather information

To give you the best care possible at your first office visit, please request any information that is pertinent from your health care team. Information that may be pertinent includes:

  • Blood work and other labs done within 1 year- Always request these if you do not have personal copies

  • Imaging- If done within 3 years and is pertinent to your case

  • Chart notes- Request the last 6 months if you have been regularly seeing a provider for the same or related concerns

We're all busy! If you have forgotten, or were unable to complete the above steps, make a follow up appointment 1-2 weeks after you initial appointment so we an gather the information together and make alterations to your treatment plan if needed.

CANCELLATION POLICY

You can cancel your scheduled appointment at any time before 48 hours. If you cancel within 48 hours of you appointment, you will be charged a late cancellation fee. 

Exceptions are always made for unpredictable circumstances such as inclement weather, power outages, emergencies, etc. Please contact Simply Natural Medicine directly if needed. 

If Dr. Chloe needs to cancel an appointment due to an sickness, inclement weather, or an emergency, you will notified as soon as possible and be rescheduled in a timely manner.
