This is the new charting and telemedicine platform I will be using moving forward. There are no downloads necessary. You can schedule an appointment without registering for a portal using the link above. You will be invited to join the portal prior to your first visit. Within the portal, you will be able to schedule future appointments, see treatment plans, seeing billing information, send portal messages, upload files, and more.

You should have received an invite to create a patient portal. Check your email (including your junk email) for the sender “Simply Natural Medicine” or “noreply@charmtracker.com” for a link.

Insurance Coverage

I will continue to bill your insurance as usual. Accepted insurances include Regence BCBS, United Health Care, Moda, Pacific Source, First Choice, Cigna, Aetna, and HealthNet (please verify Telehealth coverage).

If you are using Oregon Insurance, choose the option that starts with “Oregon Insurance”.
If you are paying time of service, you will see the visit type and cost clearly indicated.

We require all patients to keep a card on file as part of our standard office policy. This helps ensure smooth, efficient clinic operations with a small administrative team and prevents delays in processing charges related to scheduled services, administrative fees, or other agreed-upon care.

Patients will always be informed in advance before any payment is processed, and we do not charge cards without appropriate notice. If there is ever a concern or discrepancy, we will communicate directly and promptly correct any issues.

Having a card on file allows us to reduce administrative overhead and focus more time and resources on patient care rather than billing follow-up, while also ensuring continuity and consistency in our practice operations.

I will continue to have access to your medical records through Kwan Yin. If you would like to have your medical records sent elsewhere, you will continue to be able to contact Kwan Yin for medical records requests and releases. Contact information remains the same. To call: 503-701-8766.
