First: Book an appointment
A new patient visit is 60 minutes. Ways to book an appointment:
Call 503-701-8766 to speak to the new patient coordinator.
Second: Fill out paperwork
Schedule a new patient appointment and you will be prompted to fill out paper work through the portal prior to your appointment.
Third: Gather information
To give you the best care possible at your first office visit, please request any information that is pertinent from your health care team. Information that may be pertinent includes:
Blood work and other labs done within 1 year- Always request these if you do not have personal copies.
Imaging- If done within 3 years and is pertinent to your case.
Chart notes- Request the last 6 months if you have been regularly seeing a provider for the same or related concerns.

We're all busy! If you have forgotten, or were unable to complete the above steps, make a follow up appointment 1-2 weeks after you initial appointment so we an gather the information together and make alterations to your treatment plan if needed.

You can cancel your scheduled appointment at any time before 48 hours. If you cancel within 48 hours of you appointment, you will be charged a late cancellation fee.
