I’m excited to continue caring for my Oregon patients as I transition my practice from Kwan Yin Healing Arts into my own private practice. As part of this change, I am now using a new electronic medical record system called Charm, which is designed to be very user-friendly, and I’m hopeful it will make for a smooth and easy transition for everyone.

When scheduling your first appointment, you may be asked to provide a credit card on file. Your card will not be charged at the time of your appointment. It is kept on file for applicable charges such as copays, deductibles, or any services not covered by insurance. You will always be notified in advance before any charges are processed.
Prior to your first appointment via Simply Natural Medicine, you will receive an email requesting you to fill out forms:
Clinic policies and financial agreement
Telemedicine consent
Authorization to bill insurance (if using insurance)
Credit card authorization
Most importantly, there will be no changes to your care—you will continue to receive the same thoughtful, patient-centered, and thorough care you have always received. If you have any questions during the transition, please feel free to email me at info@simplynaturalmedicine.org.

Schedule a new patient appointment below and you will be prompted to fill out paper work through the portal prior to your appointment.
To give you the best care possible at your first office visit, please request any information that is pertinent from your health care team. Information that may be pertinent includes:
Blood work and other labs done within 1 year- Always request these if you do not have personal copies.
Imaging- If done within 3 years and is pertinent to your case.
Chart notes- Request the last 6 months if you have been regularly seeing a provider for the same or related concerns.

CANCELLATION POLICY
You can cancel your scheduled appointment at any time before 48 hours. If you cancel within 48 hours of you appointment, you will be charged a late cancellation fee of $75.
Exceptions are always made for unpredictable circumstances such as inclement weather, power outages, emergencies, etc. Please contact Simply Natural Medicine directly if needed.
If Dr. Chloe needs to cancel an appointment due to an sickness, inclement weather, or an emergency, you will notified as soon as possible and be rescheduled in a timely manner.
